Advance Care Planning is a process of knowledge sharing and planning between patients or their decision makers and the health care provider to prepare for future health care decisions in a way that reflects the person's values and aligns with the health care consent act.
Sandy Shamon, MD, CCFP (PC)
Dr. Sandy Shamon obtained her MD at UBC and trained in Family Medicine and Palliative Care at Western University. She practiced as a family physician in Cambridge Ontario before moving to Toronto to focus her practice in palliative care and LTC medicine. Dr. Sandy Shamon is the President with OLTCC (Ontario Long Term Care Clinicians). In addition to her clinical practice, Dr. Shamon holds academic affiliations at the University of Toronto and McMaster University and is involved in academic research and teaching medical students and residents. In these roles, she hopes to put into practice her passion for enabling person-centered care and decision making.
