Join us at the Primary Care Summit to help shape and transform the future of primary care in our region.
This summit is an exciting opportunity to bring together all primary care family physicians, nurse practitioners, allied health team directors and providers to share ideas, and spark innovation on the future of primary care in Frontenac, Lennox and Addington. Dive into meaningful collaboration and gain practical tools to support health-care providers in transforming care delivery.
The summit will provide an update on the development and implementation of the Health Home model and seek valuable input from health-care providers on transitioning existing practices into People-Centered Health Homes. The event will also engage attendees on how to attach all residents of Frontenac, Lennox and Addington to a primary care provider.
We are excited to welcome our keynote speaker Dr. Renee Fernandez, Executive Director, BC Family Doctors, and other special guest speakers from CarePoint Health and Ontario Health who will all share their insights into innovation and the journey toward People-Centered Health Homes.s.
By the end of the summit, you will:
