Transition to Adult Care Workshop

St. Pauls’ Hospital in Vancouver, BC offers workshops for youth and young adults (ages 16-25 years), and their parents/guardians, to learn more about the transition to adult care. This workshop is co-facilitated by young adults who have lived experience with health care transition and is designed to be both interactive and educational. This workshop is open to any youth with a health condition (it is not health condition specific).

All workshops are offered virtually over Zoom. We look forward to having you join us!
